INDEMNITY WAIVER - DFT

By signing this form, I acknowledge and agree that:

I am voluntarily participating in the Deadly Fitness Test event.

I understand that participation involves strenuous physical activity and carries inherent risks, including (but not limited to) injury, illness, muscle strain, heart complications, dehydration, and other potential health risks. I confirm that I am physically fit and have no medical conditions that would prevent safe participation.*

I have consulted with a medical professional if I have any health concerns related to exercise or high-intensity fitness activities. I understand that it is my responsibility to stop or withdraw from the event if I experience any pain, discomfort, or symptoms of distress.*

I hereby:

• Release and discharge Sweaty Palms Fitness Club, Deadly Dozen, event organizers, sponsors, staff, volunteers, and affiliates from any and all liability for injury, loss, or damage that may occur as a result of my participation.

• Agree not to hold the hosts or their representatives responsible for any accident, injury, death, theft, or loss sustained before, during, or after the event.

  Accept full responsibility for any injury or damage caused to myself, others, or property during the event

MEDIA CONSENT:

I grant permission for photos, videos, and recordings taken during the event to be used for marketing, social media, or promotional purposes by Sweaty Palms Fitness Club and Deadly Dozen.

I understand that no financial compensation will be provided for the use of these materials.*

GENERAL AGREEMENT:

• I agree to follow all event rules, safety instructions, and directions given by event organizers and staff.

• I understand that failure to comply may result in disqualification or removal from the event.

• I understand that entry fees (if applicable) are non-refundable.

• I acknowledge that this indemnity agreement is binding.
